Health screenings
Each year as per Florida Statute 381.0056, schools will offer and perform mandated, non-invasive health screenings to all students in Kindergarten, First Grade, and Third Grade. Individual students from other grades may also be referred for screenings as needed. These screenings are vision, hearing, height and weight (BMI), dental and scoliosis. Parents may choose to opt their students out of these screenings by providing written notification to the school nurse via email or handwritten note, or by completing the Health Services Program Annual Health Screenings Opt-Out Form and submitting it to the school nurse.
Opt-Out Forms may also be completed within the Annual Updates that parent/guardians complete in Skyward Family Access. If you do not have a Skyward Family Access account, you can set one up online or by sending an email and providing: your full name and a photo ID; your student’s full name, date of birth, grade level and school. A Skyward Family Access account also gives you access to your child’s grades, test scores, attendance records and other important information.
